Job Description COOK Job Function: Responsible for the preparation of all meals, purchasing foods supplies and utensils, ensuring foods are                                prepared, served, and stored by following proper sanitation practices, and evaluating individuals nutritional                                status by following policy and procedure. Supervises: No One Reports to: Qualified Intellectual Disabilities Professional (QIDP) Primary Duties: 1. Ensure that all meals are nutritious, palatable, satisfying, and prepared by following menus and standardized recipes. 2. Provide individuals with diets as prescribed by their physician. Maintain current diet cards for all individuals. 3. Maintain necessary food service records as specified in Policy 8.01. 4. Complete nutritional assessments for all Individuals during admission. 5. Review Individual information and complete a nutritional documentation form on a quarterly basis, or as needed. 6. Complete food inventory prior to going to grocery store and also prior to ordering from approved vender per Policy 8.01. 7. Purchase food and supplies from approved vender per Policy 8.01. 8. Ensure that all foods are prepared, served, and stored following proper sanitation practices as specified in state and           federal standards. 9. Follow all facility policies and procedures and be available for the monthly food service management and dietary           consultations. 10. Perform other duties as requested by the Supervisor. Qualifications for the Cook shall be: Education: High School Graduate or GED Substitute for Education: None Requirement:  State Food Service Sanitation Certification within three (3) months of employment Maintain a valid Driver’s License Demonstrate functional 8th Grade literacy through standardized exam Obtain/maintain standing as an Authorized Direct Care staff for medication administration Obtain/maintain status on the Habilitation Aid Registry Maintain acceptable status under the Healthcare Workers’ Background Check Act Complete DSP/DDA training in accordance with state regulations Physical Demands:  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  The employee must occasionally lift and/or move, push and/or pull up to 20 lbs. Specific vision abilities required by this job include close vision, depth perception, and ability to adjust focus.  While performing the duties of this job, the employee is regularly required to use hands to finger, handle or feel; reach with hands and arms; talk or hear and smell. The employee is required to move frequently and occasionally required to stoop, kneel, crouch, climb, balance, or crawl.  The employee is frequently required to sit for extended periods of time. The employee must be able to assist in the evacuation of residents in emergency situations. The employee must possess emotional stability to deal with stressful situations on a regular basis and the ability to deal with illness, death, and dying.
