Job Description DIRECT SUPPORT PERSON (DSP)/ TEAM LEADER Job Function: Care of the individuals and implementation of the individuals' programs, as developed by the Community                                Support Team (CST). Supervises: No One Reports to: QIDP/House Manager Primary Duties: 1. Supervise and assist individuals in activities of daily living. 2. Implement active treatment program and document individual's progress. 3. Assist with completion of the individual’s assessments. 4. Participate in meetings and staffings as requested. 5. Clean facility in order to maintain a safe homelike environment. 6. Perform other duties as requested by a Supervisor. Qualifications for DSP shall be: Education: High School Graduate or GED Substitution for Education:None Experience:None Requirements:Maintain a valid Driver’s License Demonstrate functional 8th Grade literacy through standardized exam Obtain/maintain standing as an Authorized Direct Care staff for medication administration Complete DSP/DDA training in accordance with state regulations, and maintain status on the Habilitation Aid Registry Maintain acceptable status under the Healthcare Workers’ Background Check Act Physical Demands:  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  The employee must occasionally lift and/or move, push and/or pull up to 20 lbs. Specific vision abilities required by this job include close vision, depth perception, and ability to adjust focus.  While performing the duties of this job, the employee is regularly required to use hands to finger, handle or feel; reach with hands and arms; talk or hear and smell. The employee is required to move frequently and occasionally required to stoop, kneel, crouch, climb, balance, or crawl.  The employee is frequently required to sit for extended periods of time. The employee must be able to assist in the evacuation of residents in emergency situations. The employee must possess emotional stability to deal with stressful situations on a regular basis and the ability to deal with illness, death, and dying.
