Job Description LICENSED PRACTICAL NURSE CONSULTANT FULL-TIME LPN; PART-TIME LPN Job Function:  Implementation of medical care and services to residing individuals in accordance with                         state and federal regulations within their licensed scope of practice. Supervises:      No One Reports to:      Administrator Primary Duties: 1. Ensure complete medical and health related care needs of residing individuals as ordered by the primary care physician and all qualified consulting medical professionals is met in accordance with applicable state and federal regulations for licensure and Medicaid Certification. 2. Assist with training for DSP upon hire and as indicated to provide appropriate basic care to residing individuals in accordance with state and federal regulations for licensure and Medicaid Certification. 3. Will retain professional accountability for the outcome of tasks and all nursing care subcomponents of total patient care. 4. Complete data collection and collaborate on residing individual’s health assessments, direct medical care not indicated for authorized DSP staff, review monthly physician orders and lab results, provide concise documentation, follow up and consultation with appropriate oversight of RN, Administrator, appropriate medical professionals, and management staff during routine scheduled and PRN visits to facilities. 5. Provide basic routine medical care and participate in the evaluation of residing individuals’ minor injuries and illnesses PRN. 6. Provide immunizations to residing individuals and staff members. 7. Participate as part of the Community Support Team, Behavior Management Committee, Quality Assurance and Investigative Committees. 8. Perform other duties as requested by Administrator. Qualifications for the Licensed Practical Nurse shall be: Education:              Successful completion of an accredited Licensed Practical Nurse Program. Substitute for Education: None Experience:  Preferred 2 years of active nursing employment within the past 5 years, with 1 year                      experience in the field of Developmental Disabilities Nursing. Substitute for Experience:  As deemed appropriate. Requirements:   Current Illinois Licensed Practical Nurse License. Maintain a valid Driver’s License. Demonstrate functional 8th Grade literacy through standardized exam Maintain acceptable status under the Healthcare Workers’ Background Check Act. Satisfactory completion of Certified Medication Trainer within 90 days of employment.  Dependable accessibility 24 hours daily. Effective communication and teaching skills.
